Teton Dam Failure

The 1976 Teton Dam breach
— During reservoir filling, water seeped through an abuttment of the Teton embankment dam, and quickly eroded into the dam itself.  The dam dam is completely breached, in what may be the largest singe structural failure in U.S. history.
